Want to be referred or make a referral to S2R?
Form 1 is for mental health professionals such as psychiatrists, psychologists, Community Psychiatric Nurses, Social Workers, Occupational Therapists to complete with the person they are referring.
Form 2 is a one page referral form for GPs to use.
There are different ones for North and South Kirklees.
If you are a person wishing to be referred and have no CPN or Social Worker,
you can take this referral form to your GP.
